3. Identification Numbers
- Social Security Numbers
- Social Insurance Number
- Business Identification Number
- Employer Identification Number
- Tax ID Numbers
4. Payroll information, including:
- Paychecks
- Paystubs
- Pay rates
5. Medical/Dental Insurance Numbers
- Doctors Names and claims
- Insurance claims
- Prescriptions
- Any related personal information
6. Phone Numbers
7. Other Personal Information belonging to Customers, Employees and Contractors, examples would include:
- Date of Birth
- Address
- Phone Numbers
- Maiden Name
- Names
- Customer Number
- Invoice Number
